Description of service provided: 

Behavioral Health Home Plus (BHHP) is a comprehensive model that addresses physical health and wellness as part of recovery for adults living with mental illness. BHHP enhances the capacity of behavioral health providers in order to help people identify physical health and wellness challenges as well as activate people to become better informed, effective managers of their overall health.  This service is offered at the DCI Main Office at 43 Servidea Drive, Ridgway or within the individual’s home.

Wellness Nurse:

The BHHP Wellness Nurse works together with Blended Case Managers and the people they serve with the goal of providing valuable wellness training and support. The Wellness Nurse:

  • Accompanies individuals to medical appointments when necessary.
  • Advocates for their health by facilitating access to quality physical and behavioral health services.
  • Supports their self-advocacy efforts with healthcare providers. 
  • Offers wellness materials to help individuals manage chronic conditions.
  • Develops preventative wellness plans.
  • Organizes team meetings to coordinate care for those with complex health needs. 
  • Communicates regularly with individuals, families, caregivers, and healthcare providers to ensure priorities are addressed, training opportunities are identified, and needs are met.

Admission criteria:

To be eligible, an individual must be and Elk County resident over the age of 18 years, a recipient of Blended Case Management Services, and funded through Community Care Behavioral Health (CCBH).

How to access service:

Referral form completed by a provider, which includes documentation of mental health diagnosis and criteria.

How individuals pay for the service:

Consumers do not directly pay for this service. The BHHP program is free to consumers who meet admission criteria.

Behavioral Health Home Plus is funded by Community Care Behavioral Health (CCBH).
